- The flight from Taipei to about 300 nmi northwest of San Francisco was uneventful and the airplane was flying at about 41,000 feet mean sea level when the No. 4 engine lost power. During the attempt to recover and restore normal power on the No. 4 engine, the airplane rolled to the right, nosed over, and entered an uncontrollable descent. The captain was unable to restore the airplane to stable flight until it had descended to 9,500 feet. After the captain stabilized the airplane, he elected to divert to San Francisco International Airport, where a safe landing was made. Although the airplane suffered major structural damage during the upset, descent, and subsequent recovery, only two persons among the 274 passengers and crew on board were injured seriously.
The maximum vertical acceleration forces recorded during the descent were 4.8Gs and 5.1Gs as the airplane descended through 30,552 feet and 19,083 feet, respectively.
PROBABLE CAUSE: "The captain"s preoccupation with an inflight malfunction and his failure to monitor properly the airplane"s flight instruments which resulted in his losing control of the airplane. Contributing to the accident was the captain"s over-reliance on the autopilot after the loss of thrust on the no. 4 engine."

中華航空006號班機 摘要 日期 1985年2月19日 類型 第四引擎失效、機師操作錯誤、疲勞值勤 發生地 太平洋上空 罹難 0 受傷 2 飛機 機型 波音747SP-09 經營者 中華航空 起飛地 台灣中正國際機場 飛機註冊編號 N4522V 乘客 251 機組人員 23 生還者 274(全機生還) 中華航空006號班機 是中華航空的波音747SP飛機從台北於下午4:40起飛前往洛杉磯,表定於當地時間下午1:25到達。在1985年2月19日,該航班突然發生意外,導致機上其中兩名乘客重傷以及飛機受到相當損毀。原因是該班經過十小時飛行,機上其中一具引擎失去動力。雖然當時仍有數分鐘可以處理這個狀況,但飛行員仍因引擎失效的問題,在校正不平均的動力時失敗了。這架飛機最後不斷翻滾並以接近音速的速度下墜了9000公尺,在受到高速與高壓的衝擊之後,飛行員重新控制了飛機並轉降在舊金山。

依據黑盒子資料以電腦重建的飛機下墜時飛行姿態圖在離開台北後的10小時,該架波音747位於舊金山西北邊約350英里(550公里)的位置遇到亂流。 這造成飛行電腦調整引擎油門。而4號引擎不僅沒有加速,反而停留在低功率狀態運行。 隨機工程師試圖解決,但未成功。可能的原因之一是其忽略關閉一個放氣裝置。在1分30秒後引擎熄火,並迅速失去動力。
機長獲知引擎熄火後,指示隨機工程師重新啟動引擎(但此程序的執行要求要在30000英尺的高度之下執行),由於缺乏氧氣及高度過高,引擎無法重新啟動,而客機仍停留在41000呎的高度。機長並沒有使用方向舵來抵消來自右邊的推力,並且仍將自動駕駛儀保持在開啟的狀態。雖然處理引擎中止問題的標準程序是先使用方向舵的抵銷推力,然後解除自動駕駛讓飛機維持在適當的平衡狀態。而自動駕駛儀使飛機維持在適當的高度,然後將操縱桿轉向左方恢復至原本路線中心延長線的on course狀態,而當時客機不平衡的原因是由飛機動力輸出偏右所引起的。隨後自動駕駛儀自動調整至向左偏轉,但這項動作仍耗費了數分鐘之久。最後自動駕駛儀已經扭轉至最大極限,而飛機卻開始慢慢向右偏轉。
此時機長迅速對持續的空速下降作出調整(因為當飛機不平穩時,在缺乏動力下同時阻力逐漸增加)。啟動自動駕駛儀並降下機鼻從而增加速度,也監控著空速指示器的指數。在引擎問題發生後的3分鐘,機長仍專注於飛機速度的下降問題,並嘗試在自動駕駛的狀態下控制飛機。但仍然未察覺操縱桿朝左的自動駕駛的仍然處於運作狀態,也沒有留意客機仍在右轉。當檢查飛機姿態儀時,卻發現水平儀呈現垂直狀態。機長認為是儀器失靈所致,隨後檢查副機師的飛機姿態儀與備用的飛機姿態儀後,仍認為是儀器失靈導致出現兩個飛行姿態儀顯示同樣讀數的狀況。此時飛機進入雲層因此他也無法透過窗戶來觀察與定位。事實上當時飛機已經開始呈現翻轉以及垂直俯衝的狀態,在高速俯衝的狀態下在20秒內猛降10000英尺,此時機組員和乘客受到巨大的離心力。
當飛機俯衝至雲層底部時(約11000英尺),機師才可看見地平線,並能以目測的方式來控制飛機。他們必須在兩分半內從30000英尺下降到9600英尺。全機組員認為當時4具引擎都已經全部熄火,但美國國家運輸安全委員會(National Transportation Safety Board)在調查報告中指出當時只有四號引擎失效,仍然可使用其餘3具工作正常的引擎以調整角度,同時機組人員也嘗試重新發動四號引擎。他們開始爬升並給航空交通管制中心回報狀態正常,並將繼續將飛機開往目的地洛杉磯。隨後他們發現起落架已經放下後,飛機其中一個液壓系統壓力為0。而起落架放下後所增加的阻力使客機無法擁有足夠的燃料可供降落在洛杉磯,因此他們決定轉降舊金山。雖然飛機當時已停止俯衝,但由於機長發現飛機的升降舵並不能正常運作,令飛機降落時有困難。考慮到落地時會有人員受傷,因此當局也發佈緊急命令,讓飛機直飛到機場以避免事情惡化。
由於機長得知飛機的水平尾翼已經受損,因此只可使用逐漸調校引擎的功率讓飛機降落。最後,航機終於安全降落舊金山國際機場。

在事故中有兩人受傷,一位是腿部骨折及拉傷;另外則是背部嚴重拉傷需要入院治療。而飛機則是在事故中已經嚴重受損。主機翼向上彎曲兩英吋。起落架與機身部分受損。損壞最嚴重的是機尾,大部分水平尾翼已經折斷。整個左側的升降舵與管線完全斷開,供應此處動力的液壓系統也已經破裂並且嚴重耗盡。起落架的艙門也在飛機俯衝時扯脫。

經過連續不斷的調查後在報告結果最受到國際注意的部分,是事件問題肇因於飛行員在高度疲勞下所發生的操作錯誤。雖然機師相當具有經驗並且在兩週前也駕駛過六趟國際航班,在測試時機長也認為自己並不勞累,但 NTSB 調查員認為機長在休息時間並未補充睡眠,且事實上在事件發生的當時是台北時間的半夜兩點鐘。這使得他在飛機事故發生時判斷錯誤及沒有能力處理各項細節問題,才導致無力避免事件的發生。

此架飛機在修復後仍繼續使用,一直到1997年,華航的子公司華信航空將此機退租。幾經轉手後,根據報導這架飛機已經很久未飛行處於閒置狀態,正待售當中。[1]

波音飞机自动驾驶不控制方向舵,当时一发失效,两边推力不平衡了,产生偏转,飞行员仍然使用自动驾驶,但自动驾驶仅能通过操控副翼保持不横滚,这期间,飞行员因为疲劳始终没有想起来蹬舵找平衡,当脱开自动驾驶后,情况糟糕,不过还好最后控制住,平安降落了~

- 当一个发动机失效时,三个人(机长,副驾驶,机械师)没能立即按照检查单进行应急处理,而是忙于在包线外启动发动机,造成飞机侧滑和偏转加剧,直至进入螺旋,而后三个人一直认为是姿态表故障,不相信仪表,当时好像是韩国机长,说跨时区和大强度飞行带来的疲劳,影响了他的判断力,幸运的是只有飞机几乎损毁没有人亡-------百年世纪空难有介绍

- 这个培训的时候看过, 因为是疲劳驾驶, 导致disorientation(中文叫什么?)当一发出错时,未能按检查单程序检查,导致连环效应的失误。

这个视频上说B747原来的自动驾驶仪不控制方向舵(我没干过B747,不知道到底怎么样,要是说错了的话,业内人士去拍视频里那些专家,别拍我啊
),要真是这样的话,感觉机长关闭AP关的太晚了,当时开始向右侧偏转的时候,断开AP,把方向舵向向左边做一下配平,同时加大另外三台发动机的推力就应该不至于产生那样的后果了......当然了,这是在家里坐着打字,典型的"站着说话不腰疼"
!不过,我到现在也没搞明白,三个飞行员怎么会同时认为两个主显示器和一个备用地平仪同时出故障和三台发动机同时停车呢??
这三位仁兄当时同时都困到什么地步了

The roll up stairs were a little too short and some 8 feet too low to allow those on board to exit. Ground crew on the ramp were saying there never has been a Boeing 747 on the airport before. Judging by the landing and roll out, the aircraft must have been extremely light. The aircraft was slowed to a safe taxi speed at the 4500 foot position on the runway.

- 台湾华航真是安全差,我记得还一个747在台湾海峡上面平飞时解体了,谁知道是怎么回事?据说是撞上流星了。
